About our Fees
Heal and Thrive operates as a small, private clinic, and all services incur an out-of-pocket fee. You’ll find a detailed schedule of fees to the right (click each appointment type).
Depending on your circumstances, you may be eligible for partial reimbursement through the following funding schemes:
Medicare - If you have a current Chronic Disease Management (CDM) Plan from your GP
Private health insurance extras cover (check with your insurer for eligibility and rebate amount)
NDIS (Self-managed or plan-managed)
Approved WorkSafe Victoria claims
You will find detailed information about various funding schemes further down this page.
If you’re unsure about your eligibility or funding pathway, feel free to get in touch for support.

-
Rebate amount: $60.34 per session, for up to five sessions
Eligibility: Requires a Chronic Disease Management Plan (CDM) from your GP
Medicare Item Number: 10953 (Exercise Physiology)
Medicare rebates may be available for clients with a valid Chronic Disease Management Plan (CDM) from their GP.
At Heal and Thrive, we are unable to offer bulk billing, as the current rebate does not adequately reflect the time, planning, and support involved in your care.
Payment in full is required within 7 days from the date of the invoice or prior to your next appointment.
If you are eligible, you can then claim a Medicare rebate, which will partially reimburse the cost of your session and reduce your out-of-pocket expense. Up to five Exercise Physiology sessions can be claimed per calendar year.
If you’ve reached your Medicare Safety Net threshold (original or extended), your rebate may be higher — in some cases, covering up to 80% of the fee.
Please note: You cannot claim both a Medicare rebate and private health insurance for the same service.
Further information about CDM eligibility and rebate amounts by visiting the Medicare website.

NDIS Billing Code: 15_200_0126_1_3 (Exercise Physiology - Improved Daily Living or Improved Health & Wellbeing)
Heal and Thrive welcomes NDIS participants who are either self-managed or plan-managed, provided their plan includes suitable funding for our services. At this stage, we are not registered with the NDIA, so we are unable to see agency-managed participants.
Signed service agreements are required for all NDIS participants.
Payment in full is required within 7 days from the date of the invoice or prior to your next appointment.

Late Cancellation and
Non-Attendance Fees
We kindly ask that you provide as much notice as possible if you need to cancel or reschedule your appointment. You are welcome to do this via any means of communication that suits you (text, email or phone call). This helps us offer the appointment time to someone else who may be waiting for care.
If less than 48 hours’ notice is given, or if you do not attend without prior notice, a cancellation fee will apply.
